Complete and submit this form to register your organization with the 211 National Data Platform. Participation in the platform is voluntary, and you may remove your data from the platform at any time.

Enter all required information to register an organization and administrator account for transmitting data to the national platform. By submitting this form, you agree to have United Way Worldwide ("UWW") securely store a copy of your 211 data in the national platform, in accordance with all data governance and management policies.

After submitting this form, an email will be sent to the person designated in your organization to receive platform communications, including instructions for uploading your organization's data to the platform.

^✱ What country is your organization located? We use this information to ensure that your files are securely stored in your country and configure access to services.

^✱ Which 211 Data Management system does your organization use?

^✱ Enter the first name of a person in your organization who has permissions to export data from your data management system (e.g. Fred)

^✱ Enter the last name of a person in your organization who has permissions to export data from your data management system (e.g. Flinstone)

^✱ Create a name for your organization. The name must be less than 25 characters (including spaces) and should only include characters (a-z), numbers and spaces. Choose your name carefully because it is used not only to configure administration settings, but also to attribute your organization to data your organization contributes to the National Platform. TIP: We suggest using the name of the city where your 211 center is located, prefixed with '211' (e.g. 211 Miami).

^✱ Enter your organization code. An organization code is used to verify your organization's request to join the National 211 Data Platform. Your code was emailed to you after you submitted your request to create an account on the platform. If you have not yet requested a code, click here to go to the 'Request a Code' page. If you have lost or forgotten your registration code, contact us by email at support@211technology for assistance.
